What about running back to a station to relaod then returning to the battlefield only to realize that you forgot to reload, then go back again and come back without reloading again, then repeating so a third time :P
i think being so intent on shooting and dodging your target you do not realize he has positioned you to slam yourself into a roid.

getting in a bus duel but your enemy not realising it was an any loadout bus duel..and him still winning vs me in a nuet fast charge...and him still being on 40%.....that was not my proudest day.
Being in a fight against someone I've never beaten, actually winning, then getting flustered and hitting a series of caplocks, tilde, etc.
But having the good grace not to make excuses after loosing, because I did press the buttons myself and lost :)

...Engaging someone outside the station in Odia M-14, and then realising 30 seconds after the first shot hits, they put a firezone at said staion (I lasted about 3 seconds after I realised, it was too late :P, heh) oh, and that person turned out to be a Dev. hehe

I think the most embarrassing thing to happen in PvP is two players using all-ammo loadouts *both* running out of ammo in a duel. :D
I'd say infiltrating a guild then double crossing the person who hired you only to accidently /msg him saying he is falling for it. :)

I was dog-fighting a pilot who was using back and roll tactics. The pilot was backing right towards the middle of a huge roid. I kept pushing the pilot back while watching the roid fill my HUD. Must have be embarressing to the pilot when his ship went smack.
Definately the most embarassing thing is a roid-smack to end a battle...BOOM...wtf was that...

I died well over 600 times playing this game, so I consider myself an expert.
I'll have to agree - smacking into a roid when fighting is kinda silly. Done that.
Although dying from your own splashdamage is kinda silly, too. I've been in a damaged Rag, drop my ordanance load, follow it in to make sure everything hits, then either
a) hit the capship, or
b) catch some splash from my own swarms and poof
I've had my daughter pull the controls out of my hand when fighting people before as well. (She liked to watch daddy shoot badguys) Suddenly I'd stop shooting and drift for a bit. Then I'd usually die. She'd say "Awwwww. Did you DIE? That's OK dada."
Kids definitely give you perspective on life, that's fer sure.
Although the BEST way for me to have died was:
LeberMac, late for the game as usual, heads for the capship docking bay as the capship is about to warp, and... I'm right in front of it when it warps, it smacks me really good and I careen into an asteroid.

Although Apex can tell you about the time I killed him with concusssion mines in Sedina D-14. Me in a moth, he in a bus, we could not damage each other. But my conc mines definitely could shoot him into an asteroid at 2000 m/s. Hehe. I think Niki witnessed it.

I was in the middle of a duel and all of a sudden my ping goes through the roof and everything just starts free floating.
I thought "uh oh..."
Sure enough, my little brother was messing around with the router that was located in his room and ended up unplugging the ethernet cable that led to my computer down the hall. Talk about bad timing.
The second I plug it back in my ship goes BOOOM.

Nah, best way was when I tried to intercept a moth in my rev C, and ram it off course, it was going full speed, fully loaded with phase arrays... I got in front of it...

kernel, it was me you were fighting. It was really funny, I see this ice roid coming up, and kernel looks like he's headed behind the roid, so I change my aim to catch him when he appears again, and all of a sudden, I see an explosion coming around the roid where he disappeared.
I was fighting a vulture and managed to wing bug my rev c off it. O.o

Going on a long haul to retrieve some CtC cargo, but then hitting /explode instead of /activate when you get to the destination station. And being homed in Pelatus.
