I was taking a few escort missions earlier, and I noticed that every once in a while (especially right after the voy got out of an ion storm) I would see something like "Aputech-5 Guardian KD-1234 killed [NPC name here]". Yet that NPC was certainly not killed in the storm, it's in the sector at full health!

On escort missions I also sometimes get (usually in station sector) "[NPC name here] is having issues", yet that NPC is coming right up behind me. O...k... interesting...

just in case it was supposed to be fixed, I'd like to say that it's still doing it. Only now it's mostly right after they dock and leave the group (and then it says that some hive bot kills them), which is kinda, err, strange.

It's getting better, though, I think. Haven't done many voys recently though, maybe it's just dumb luck.
