Seems to be a bug in the mission where you have to find the CEO of Axia Corp. I found the CEO, but on returning to the station I was told that I'd failed.

Messages in the logs as follows:

Good job. You found the CEO. Get to a station to report back.
The meeting got ambushed and everone[sic] ran away.
You have failed the mission.

That due to bots attacking npc's, when the mission was created, hive bots ignored npc's. The entire mission tree is buggy as hell anyway :P
