What's the upload speed on the net connection for the server, and how much traffic is there (on average) per user?
It's roughly 14kbits/user. Hmm. Updater traffic may be an issue, but we never spike above 1.4Mbits total. We're not hitting our own bandwidth cap.
It sure is finicky, though. Trying just now, I got 0% packet loss, with "round-trip min/avg/max = 60.286/61.741/65.494 ms."
What other tests could I do to help you diagnose, a1k0n? I'm not particularly versed in internet router mumbojumbo.

Huh... What's the hardware in the server? It may be dying (literally, I don't mean obsolete, since I already know it is :P)...
Well, after messing with it tonight, it looks like the update server is eating up CPU and bandwidth, causing "lag spikes". I need to focus some optimization in that direction ASAP. The update server has a soft rate limit and no hard one, so that could be a problem.
The update server being the one that scans for current versions and updates and the like?

Oh yeah, i think that several people may be lagging at the same time, because a few times when i get bad lag, other people will mention it.

Oh yeah, avg ping... almost forgot. 3423 for a while before averaging at around 50000 for a short time. At that point i quit before i could die from lag.
I noticed it to my ping went from an average of 42 to over 200! I couldn't believe it, but I've seen no latency issues between me and the server yet...
