As a selectable option during user creation, i'd actually +1 this. Users would get a small badge in /charinfo. You would have to allready have a charecter at 4/4/4/4/4 before the option was visible.

Yeah, genka is right, I've done it too. Simply told myself in my head that if I die at all the character is dead and deleted it. Others have too, see Iron Faille.

Not saying I would not like the option, but if you really do want to play a permadeath character, there is nothing stopping you.
