I'm a little hazy about what you mean by "Killed" - escape device
Okay, I'm neurotic and I worry about the strangest things. I keep getting "killed" yet I instantly appear at my home base, remarkably not dead. While it isn't wise to look a gift-horse in the mouth, I (and perhaps many others) would sleep easier if the following explanation ( or something similar ) of this phenomenon was inserted into the game universe:
Every spacecraft in this day and age is equipped with a one-shot emergency teleport device located in the cockpit, usually in the back of the pilot's chair. Now we all know that teleportation is an impractical method of transportation, due to the enormous amount of energy required to transmit matter coherently through sub-space. Fortunately for unlucky or less-skillful pilots, the huge energy pulse released by the explosion of their ship's power plant provides just enough to fuel a one way trip to the vortex-rescue station encoded in his ship's computer with minimal exposure to the risk of arriving as a blob of formless protoplasm. Therefore "Killed" refers to vessels "killed",not pilots.

the general consensus is a snapshot is taken of your brain right when you die and you wake up in a clone bay. problem solved
it turns out you are actually a robot.
We live . . . again!
I allways wanted to more details of this side of VO's history...
Maybe we all "download and ressurect" using "organic memory transfer" process.
But that would affect bounty hunter, war aspects, so on. None of VO missions even consider this openly, but some allow you to continue if you die, while others end on death.
And there are better things to show about it. Instead of lelecter's image, I prefer this:

This is probably the only time I'll ever say this, but, I agree with Alloh. Though if it was Grace Park waking up naked in a tub, I'd prefer it even more.
