Local temp kill bug
I recently allowed Rin to kill me in the NFZ of Odia H6 and did not forgive the kill when prompted.
When I returned to the sector a few minutes later to pick up my cargo the strike force attacked me.
I did not bump or fire on Rin or the strike force.
A local Temp KOS should not have been assigned.
The KOS should have been removed when I left the sector the first time.

I probably got too close and got hit by the explosion. If that's the case this might be related to how if you are homed at a station and hurt somebody when you die inside the nfz and then undock, they immediately gank you.
This sounds like a feature, not a bug.
So you weren't homed at the station, PaK? Sounds like a bug to me...
Its happened both ways but I wasn't watching for it earlier. This time I was homed elsewhere.

This bug is still very much around.

Also it appears my attacker was not penalised for killing me in an NFZ....

Yes I was. Grey stations are just more forgiving than nations. Two kills dropped me from admired to neutral.
Leaving rats free to shoot when they want and POS characters almost helpless to defend ourselves. Makes me wonder what the point of the faction penalty is...
Anyway I think the bug is a result of the sequence of events.
If the player dies and the result damages someone else the faction system issues a temp KOS.
Since the player is in the entering station screen they do not not receive the notice and since the KOS is issued AFTER their death the server is not notified that they departed the sector leaving a temp KOS waiting for them when they return.
One fix might be to erase all Temp KOS's at the end of the player resurrection process.

Almost helpless? Bull. Carry weapons, get an escort, run away, or pay. And I'm not "free to attack you". I do get a penalty even if it isn't an insta KOS, so I can only kill a couple people in quick succession before I have to stop and repair my faction unless I want to risk becoming hated, which is a heck of a lot more work to fix since I wouldn't be able to dock to do procs. Repairing non-hated faction efficiently involves proc missions asking for goodies which have their nearest points of sale a good four wormholes away. Even when I have a healthy supply of hundreds of proc goods stockpiled ahead of time (I didn't last night) it still takes quite a few undock-dock sequences to complete the missions.
Furthermore, my need for those goodies results in additional player-to-player trade, because I sure as heck ain't gonna haul all that crap myself. And I treat my minions well. They are very fairly compensated, and get complimentary "paid" status with CHRN to boot. It's a great gig for moth-owning newbies in particular, as they tend not to have much money to play with yet. Vets tend to have so much money stashed away that they aren't really interested in selling goods on the black market, other than the really rare stuff. Their loss. Would be a good way to be counted as "paid" without the injured pride of actually paying - indeed, they'd be getting paid. And trading, which is kind of the point of being a trader?
I do agree about the temp-kos bug though, that needs to be fixed. It can get innocents in trouble with the SF, and it can cause a long sequence of undocking, being ganked by the station gaurds within the NFZ, and having your explosion damage them so that the cycle keeps repeating until you either manage to get beyond the NFZ before dying or escape the sector. (That has happened to me, though I probably deserved it :))

Er... yeah - and now back to the bug.
It's simple really, you are not allowed to be on fire in a no fire zone, stop getting shot! :P

Maybe make it so you exploding doesnt trigger nfz? kinda like how hitting yourself with a rocket doesnt trigger it any more...
