I completed the 'Corvus System Recon' mission and had the 'High Risk Courier Needed' mission appear. I then logged out thinking the HRC mission would be there when I got back as there was *nothing* in the previous mission logs that said anything to the contrary. But when I logged back in the HRC mission had vanished into thin air.
I then did some searching and found this - High Risk Courier Needed
But does logging out and then back in again count as an HRC mission failure even though I didn't get a chance to start it? And why does the mission disappear in the first place if you log out, that doesn't make any sense as the other things you get for completing the 'Corvus System Recon' mission don't.

No, you're most likely seeing a bug that relates to missions which have time-usage limits and the new "all missions everywhere" system. You can only fail the HRCN mission up to 5 times before it vanishes, and you can only take the mission once every four hours. But if you never took the mission, and the availability disappeared, that is almost certainly a bug.
Of course, if your standing with UIT dropped significantly, that could also cause the mission to become unavailable. I assume you did not change your UIT standing in the timeframe above?

1. Hive Research (Completed)
2. Corvus System Recon (Failed)
3. Corvus System Recon (Completed)
4. High Risk Courier (Failed)
5. Corvus System Recon (Completed)
...at this point the High Risk Courier mission became available again but I logged out.
...Logged back in later the same day to find the HRC mission gone.
It's been well over 24 hours since the HRC mission vanished, it and the CSR mission are still missing.
My UIT standing is +976 but if it dropped any it could only have been by a small amount.
I don't know if it helps any but the mission to manufacture the Corvus Widowmaker is available.

I had a problem where I failed the High Risk Courier mission as well (hit an ion storm and it counted that as a failure when I left since the NPCs spawned in the storm) and it hasn't re-appeared since.
Should I raise a bug report for this or submit a support ticket?
The new "all missions everywhere" system broke many things in PCC mission "player", some reported in PCC area... Inc, plz check?
