If this is pretty universally-liked, any thoughts on what faction might sell it, or locations, or other requirements / factors to get it?

Orion because it's primarily going to be used on a Rev C. Then you can rename it something a bit more interesting than "Mk3".
As far as how to get it - best thing would be to make it manufacturable (to encourage more than just combat with the additional weapon). But that obviously also requires a mission/tree be made...

My thinking was to either very slightly improve the accuracy for all flechettes across the board and potentially increasing the damage of the higher tier versions.
Another possibility would be to have two projectiles being fired at once instead of just one, but I'm not sure if that's possible by changing one simple value in your fancy weapon stats editor... instead of being more involved.

I never fly one and have primarily used flechettes on centurion mk1's, corvus vultures and corvus marauders. Flechettes are generally too shitty to be worth manufacturing and honestly I almost never use manufactured equipment because it is such a pain in the ass to do.
If you're going to be making this a faction variant, I suggest Xang Xi because they don't really have any faction specific stuff on offer yet.

Inc, make it an Ineubis weapon with level and faction requirements similar requirements to the raven.

Have them be manufacture-able in Dau L-10 and Odia M-14. Sol II and Itan J-11 also if you like.
I'm a fan of more manufacturable things, but we don't know if this gun is going to be any good. We know it's going to be fun to play with, but it should be a regular item as it's not original enough to justify manufacturing and the scarcity that comes with it.
The problem is that if it is released first as a purchasable item and then made manufacturably exclusively later on, the market for it will be fucked from the beginning (as people will just stock up mass quantities prior to the change).
On the flip side, if it's not good enough to be an exclusively manufactured item, it's debatable whether or not it's worth introducing it at all.
Personally I do think it will be a very good item. It's basically TWO flechette mk2s (if chain-fired) in a single small port with higher damage, higher velocity, and lower weight. An industrial strength sand-blaster that will enjoy increased use in close-combat situations.
Either way, glad it's being considered.

Make it a bot drop from those ganking assault bots and upgrade their own weapons to use them. It's so funny when they kill newbs.

+1 to making the weapon provided the requirements do not follow the excessively onerous VO standard of manufacturing.
Bonus points if do this in such a way that it can be eventually incorporated into the prototype economy being tested in N2 such that the availability and pricing are driven by player activity. Not so much in the sense of player to player trade, but in providing a service to a station needing the components to build the weapon. The player could then be given either a tithe of the finished product, offered the entire batch at a discounted rate or offered premium compensation for the work done. If the player takes the compensation, the weapons would then be available for sale at a price of about 10 - 20 % of the compensation the building made for their services.
